- BAMS(UG)- Degree/Convocation Certificate.
- Income certificate/ IT Certificate of Parent.
- Registration Certificate.
- BAMS All Year’s Marks Cards.
- Transfer certificate (TC)/Leaving Certificate.
- 12 passport size photographs with white Background.
- Aadhar card of the Candidate and Parents.
FOR OUT OF STATE STUDENTS (NON-KARNATAKA STUDENTS):
- Students of other state should attach an Eligibility Certificate (EC) from Rajiv Gandhi University of Health Sciences, Bangalore, along with the application.
- Migration certificate
